On Iwas appointed to conduct an Army Regulation (AR) I5-6 investigation and a Line ofDuty investigation relating to the apparent suicide of PFC Jeremy D. Campbell l94"` Militagy Police Company, Militpgy Police Battalion. Fort Campbell, KY. Outlined below are my findings and recommendations. 2. On the momin ofMonday, 20 September 2010, PFC Jeremy Campbell was found in his car by Ofticer ofthe Oak Grove, KY Police Department near 4lA and Gate 7 Road in Oak Grove, KY. He was dead of an apparent self-inflicted gunshot wound to the head by a .22 . caliber rifle, PFC Campbell was supposed to complete a I2-mile road march that morning, but tailed to report for 0345 formation. 3. PFC Campbell had been identified as 'high risk' after an episode occurred on I September 20] 0, in which the SM?fIed Fort Campbell, KY after receiving a verbal counseling from his platoon chain of command for failing to complete a l2--mile road march in less than the Air Assault time standard of three hours. PFC Campbell's actions on 20 September were consistent with a medical intake interview conducted on 2 September where SM told a Adult Behavioral Health counselor that he had planned to "buy a gun and blow my head off" SM had generally i succeeded in all, tasks assigned to him and was thought of as a good Soldier. He was awarded an AAM in AIT for being the distinguished honor graduate. I believe the fact that he had not succeeded in the I2 mile road march, drove him to this severe and final conclusion. SM had no known financial, legal, or relational issues at the time of his clcath. SPECIFIC FINDINGS a, Military Background: (1) At what stage ofthe unit's ARFORGEN cycle (Reset, Train/Ready, Available) did the death occur'? - PFC Campbell's death occurred during the Train/Ready Stage ofthe unit`s ARFORGEN cycle. (Exhibits and J) (2) What was the ucnit's (officers, NCO's, Soldiers) overall impression Campbell? SUBJECT: Rerport ofAm1y Regulation 15-6 and Line of Duty Investigations, PFC Jeremy D. PFC Campbell was thought of as a very dependableiSo1dier. He was a good Soldier that did not have a history of any written disciplinary actions. PFC Campbell was thought of as a motivated Soldier with a good attitude. (Exhibits there evidence that PFC Campbell was being harassed or "singled out" by members ot` the l94'l` MP Company? lf so, what was the nature of the harassment (racial, sexual, nationality, etc.) and who was involved? No, there is not any evidence that PFC Campbell was being (harassed or "singled out" by members ofhis unit. (Exhibits and K) (4) Did PFC Campbell have any history of disciplinary problems? Was he recently the subject of or pending any UCMJ (Article 15, courts--martial) or adverse administrative action? lf so, describe the nature and dates of those actions. No, PFC Campbell had no history of or any other form of administrative punishment. (Exhibits and G) (5) llad PFC Campbell recently been counseled for misconduct or performance? lfso, describe the method and subject matter ofthe counseling. Ycs, PFC Campbell was verbally counseled on 1 September 2010 for failing to complete a l2 A mile road march in less than three hours which is the standard to enter Air Assault School. The counseling consisted ofthe squad leader (bl (6) xpressing his disappointment with PFC Cam bell not meetin the standard. According to the platoon leadership, (bl (6) and his counseling was not abusive or punitive in nature. (Exhibit G) (6) When was the last time PF Campbell attended suicide prevention training or briefing? PFC Campbell attended suicide prevention training on 16 August 2010 presented by Mr. Varney. (35 days prior to incident) (Exhibit R) (7) Had PFC Campbell previously deployed and what were the dates and locations of each deployment? How many months was his most recent deployment? When did he return home? Were there any warning signs during his deployment? PFC Campbell never deployed. Fort Campbell, KY was his duty assignment and he reported in May of20l 0. (Exhibit AA) b. Personality, Lifestyle, and Relationships: (1) What were PFC Campbell's personality traits (relaxed; intense, withdrawn, outgoing, etc.)? PFC Campbell was a laid back and quiet Soldier. He did not disclose much about his private life to others. (Exhibits Hand K) (2) How did PFC Campbell spend his {ree time during deployment and at home? 2 SUBJECT: Report ofAm?1y Regulation 15-6 and Line ofDuty Investigations, PFC Jeremy D. i PF Campbell never deployed. PFC Campbell lived in the barracks and spent most of his time playing computer games, playing XBOX with his roommate, and reading books. PFC Campbell 5 also enjoyed watching movies. PFC Campbell occasionally went to Nashville with his hiends and would volunteer to be the designated driver. (Exhibit J) i (3) Was PFC Campbell experiencing any financial or civilian legal problems? Was the chain of command aware, and what, if any, action was taken? No. There is no evidence that PFC Campbell was experiencing any financial or legal problems. (Exhibit and N) (4) Had there been any recent changes in mood or any of mental illness? lfso, describe when and how the changes occurred. Yes. (6) PFC Campbell's roommate, noted that PFC Cam bell's motivation and confidence began to diminish once his former squad leader went to a new unit in June or July 20lO. No one else identified any recent changes in SM's mood or any of mental illness. (Exhibit M) (5) Did PFC Campbell's behavior (eating, sleeping, drinking, drug use, gambling, tighting, spending, etc.) change prior to his death? If so, describe when and how the changes occurred. No Soldiers noticed any change in his eating, sleeping, substance use, etc. However, (P) (6) (P) (6) his Platoon Sergeant, recalls that PFC CampbelI's performance and motivation began to decline around the beginning ot`August. He started to show up late for work and would have an attitude toward his team leaders. (bl (6) his Team Leader, noted that prior to September, PFC Campbell began to have a lack of interest in many things that he did. When questioned, PFC Campbell would say there was nothing wrong. (Exhibits and J) (6) Campbell have any significant health problems? Ir" so, describe the nature and treatment of those problems. Prior to I September ZOIO, PFC Campbell did not have any significant health problems. (Exhibit (7) Did PFC Campbell have a history of alcohol and/or drug (prescription or otherwise) abuse? Were alcohol or drugs a contributing factor to his death? The only documented mention ofpossible drug use was experimentation as a teen -- LSD, marijuana, Loratab, Percocet, Adderall, Robitussin pills, etc.). PFC Campbell also drank two bottles of generic Robitussin to get a buzz. Alcohol and drugs do not appear to have been a factor in PFC Campbell's death as the autopsy confirms no drugs or alcohol in the blood and urine. (Exhibits P, S, and T) c. Relationships: 1 SUBJECT: Relport of Army Regulation lS~6 and Line ofDuty lnvestigations, PFC Jeremy D. (1) What kind of friendships did PFC Campbell have (many, few, intense, casual, etc)? PFC Campbell had a lot of niends. lle would walk around the barracks to find people to talk t0. PFC Campbell did not talk much about his private life. He had a few close friends, including SPC Ortiz who he stayed with the weekend prior to his death. (Exhibit K) (2) Had PF Campbell recently experienced any significant losses (death of family member or coworker, divorce, breakup, etc.)? Were these a contributing factor to his death? No, PF Campbell did not experience any significant losses recently. His paternal Grandmother passed away in February 2010, but he was not very close with her. (Exhibit N) (3) ls there evidence that PFC Campbell was experiencing difficulties with his spouse (if married), partner, children, and/or parents? lf so, describe the nature ofthe conflict. Was PFC Campbell undergoing any form of counseling for these issues? Was the chain of command aware? lf so, what actions were taken? There is very little evidence that PFC Campbell was experiencing any type of relationship difficulties. PFC Campbell was single with no children. On 18 September 2010, PFC Campbell was spending time with his best frien mentioned that he was frustrated about a conversation he had with a friend back home. PFC Cam bell did not share information about any mips to either his best friend, or his roommate, Mr. entioned that PFC Campbell did not speak of any serious steady girlfriend and that if PFC Campbell had one, he would have probably told him about it. (Exhibits and N) d. Mental Health and Medical History (Communication of Suicidal Intent/Prior Attempts): (1) Had PFC Campbell threatened suicide in the past? If so, to whom did he make these threats and how did he make them? What responsive action, if any, was taken and by whom? Yes. On I September 2010, PFC Campbell sent a text to his mother that he was going to end his life. On September 2010, after failing to complete a 12-mile road march in less than three i hours, PFC Campbell did not report to work call following physical fitness training. 'l`he l94"' MP Company chain of command requested law enforcement support to "ping" PFC Campbe1l's phone to determine his location. PFC Campbell was found in an hotel after ingesting two bottles ofcough medicine. PFC Campbell was ta1
